The reason for this site accessing patient GP records is:
Care Home/Hospice/Domiciliary Care/ Residential Care using GP Connect to securely view a resident's medical records which are stored at their GP practice.
Registered clinicians (for example, nurses) can access the full medical records of residents under their care. This helps them to deliver appropriate and timely care and treatment.
Non-clinical carers (for example, the care home manager) have limited access to residents' medical history to be able to safely provide continuous care. Some information they can view includes:
- Prescribed medications
- Allergies
- Recent interactions with GP
Further queries about the reason for a patient's record being accessed should be directed to the care home manager or equivalent.

GP Connect permits the sharing of patient data only for the purpose of direct care and is only available in England.

To enable Specialist Community Public Health Nurses (SCPHN) Health Visitors and School Nurses to to provide continuity of care and ensure they have a holistic view of a child and parent when conducting health needs assessments as part of the Healthy Child Programme. The Healthy Child Programme (0–19) explicitly calls for shared access to health information to improve outcomes and reduce inequalities
