Care Home/Hospice/Domiciliary Care/ Residential Care using GP Connect to securely view a resident's medical records which are stored at their GP practice.
Registered clinicians (for example, nurses) can access the full medical records of residents under their care. This helps them to deliver appropriate and timely care and treatment.
Non-clinical carers (for example, the care home manager) have limited access to residents' medical history to be able to safely provide continuous care. Some information they can view includes:
- Prescribed medications
- Allergies
- Recent interactions with GP

Katharine House Hospice requires access to the National Data Sharing Portal for GP Connect as part of its role in delivering specialist palliative and end-of-life care across Stafford and the surrounding communities. Access to shared care records through GP Connect will support safe, timely and coordinated care for patients who are often complex, clinically vulnerable and receiving input from multiple providers across primary care, community services, acute hospitals and the hospice. Hospice clinicians require access to relevant GP-held information to understand a patient’s current diagnoses, medications, allergies, recent consultations, care plans, anticipatory prescribing arrangements and wider clinical context. This access is particularly important because Katharine House Hospice provides care across a range of settings, including the Inpatient Unit, community palliative care, Hospice at Home, outpatient services and a 24/7 specialist palliative care advice line. In many cases, decisions need to be made quickly to prevent avoidable deterioration, reduce unnecessary hospital attendance or admission, and support patients to remain in their preferred place of care. Using GP Connect via the National Data Sharing Portal will improve continuity of care, reduce reliance on patients and families repeating sensitive information, support safer clinical decision-making, and strengthen collaboration between the hospice, general practice and wider health and care partners. Access will be used only where there is a legitimate relationship with the patient and where it is necessary for the provision of direct care, in line with information governance requirements, role-based access controls and organisational policies. Katharine House Hospice is therefore selecting “shared care records” as the reason for access because GP Connect will enable hospice clinicians to view relevant shared clinical information to support integrated, patient-centred care across organisational boundaries.
