Shared care
Although Harbour is a charity, we are commissioned by the Local Authority (PCC) Public Health department to deliver Drug and Alcohol services on their behalf as part of their public health responsibilities. In line with Health and Social Care Act local authorities our responsible for improving the health of their local population including services aimed at reducing drug and alcohol misuse.
As part of this service delivery, we work in close partnership with Livewell Southwest and LES GPs providing provision of safe delivery of OST prescriptions. Livewell Southwest also access and use Harbour case management system Halo to record and generate scrips as part of a shared care package.
Harbour also work in close partnership with Operational Delivery Network (ODN) and University Hospital Plymouth, especially with Hepatology around Alcohol-Related Liver Disease and Hepatitis C micro elimination. As part of this partnership, we have joined up further with University Hospital Plymouth creating Drug Liaison workers within the Hospital sharing relevant information for the safe continuity of care into the community.
Therefore we feel it would be beneficial to have access up to date health and medication information from GP and hospitals as part of providing share and safe care.
