Care Home/Hospice/Domiciliary Care/ Residential Care using GP Connect to securely view a resident's medical records which are stored at their GP practice.
Registered clinicians (for example, nurses) can access the full medical records of residents under their care. This helps them to deliver appropriate and timely care and treatment.
Non-clinical carers (for example, the care home manager) have limited access to residents' medical history to be able to safely provide continuous care. Some information they can view includes:
- Prescribed medications
- Allergies
- Recent interactions with GP

Individuals moving between these pathways—or receiving integrated packages—require continuity of information to avoid fragmentation and risk. The utilisation of GP Connect strengthens our shared care record approach by ensuring that accurate, up-to-date clinical information is accessible regardless of geographical location or commissioning body. As we may accept packages of care from multiple ICBs and Local Authorities, including those outside our core operating footprint, this interoperability is essential to maintaining consistency in clinical standards and reducing reliance on delayed or incomplete information transfers.
