The reason for this site accessing patient GP records is:

At Doccla we’re working closely with ICBs and Primary Care Networks in England to deliver a proactive service that consists of the following key features for patients:

1. Regular Doccla clinician-led assessments, health coaching & medication optimisation

2. Doccla clinician-created goals & actions that are individualised to each patient and integrated into the platform (ie available in the app)

3. Targeted patient education material (videos, reading material etc)

4. Individualised care planning

Proactive Virtual Care brings together a suite of technologies and services to optimise the healthcare of at-risk populations within their own home, avoiding emergency episodes, improving clinical outcomes, reducing healthcare costs and delivering a markedly better patient experience. Patients are identified using an AI Risk Stratification tool that connects directly to Doccla. Their vital signs, symptoms, medical history and local care provision are then factored into the creation of a care plan. This care plan will contain elements of health coaching, remote patient monitoring, telemedicine and medicines optimisation - several of these elements can now be delivered at a price point that delivers excellent health economic outcomes through leveraging the latest in Voice LLMs. This tech-enabled care model also acts as an integration layer, bringing together patient-derived information not only from Virtual Wards and Remote Patient Monitoring, but also District Nursing and Social Care interactions in the home. This enables the Proactive Care team to operate as a coordinating layer, taking on increasingly complex caseloads and responsibilities personalised to those aspects of a patient’s care that will make the biggest difference to their health outcomes.

Doccla’s service model is firmly embedded within the definition of direct care, as outlined by NHS policy and GDPR. Our Proactive Virtual Care model is commissioned by NHS organisations and directly involves clinical activities that support the prevention, diagnosis, treatment, and ongoing management of patients. This includes:

• Direct interaction with patients: Doccla clinicians (e.g. nurses, pharmacists, health coaches) engage in regular consultations and assessments with patients—either virtually or in coordination with local care teams.

• Care delivery and optimisation: We provide medication reviews, health coaching, symptom monitoring, and risk-based intervention planning.

• Individualised care planning: Each patient receives a personalised care plan, created and adapted by clinicians based on their health data, ongoing needs, and treatment goals.

• Integrated multidisciplinary involvement: Our model operates in close coordination with ICBs, PCNs, and local services, ensuring aligned care pathways and direct support for existing NHS teams.

All of these activities are clinically justified, patient-facing, and fall within the scope of direct care. Doccla is not a data processor for research or secondary use—we are delivering clinical services commissioned by NHS bodies, and access patient information strictly in the course of delivering those services.

Access to the patient record will be limited to registered clinicians employed or subcontracted by Doccla, and includes:

• Doccla nurses and health coaches: For conducting assessments, reviewing observations, setting care goals, and providing education and coaching.

• Clinical pharmacists: For medication reviews, deprescribing, and optimisation.

• Service leads and supervising consultants: For care oversight, governance, and escalation management.

Reason for access:

These clinicians require access to the patient record to:

• Establish clinical context and history

• Tailor care plans to existing long-term conditions and medication profiles

• Coordinate with existing care teams (including GPs and virtual ward teams)

• Document interventions and update plans of care
