The reason for this site accessing patient GP records is:

A shared care record provides health and care staff with a selected view of the patient information held within different local health and care settings.

Where a shared care record has used GP Connect an up-to-date view of the medical record held at the patients' registered GP has been provided. The shared care record user can access information such as the list of current and historical GP prescribed medications, recorded allergies and a record of recent GP encounters – alongside data from other sources such as local hospital trust and social care systems.

GP Connect permits the sharing of patient data only for the purpose of direct care and is only available in England.

South Coast Alliance uses GP Connect and Shared Care Records to support the direct care of patients accessing NHS 111, Out of Hours services, and Primary Care Extended Access appointments. Access to relevant information from a patient's GP record helps healthcare professionals make informed clinical decisions, reduces the need for patients to repeat their medical history, and supports safe, effective, and coordinated care. Within our Extended Access service, clinicians working across participating practices may access relevant records and documents where appropriate to provide treatment and continuity of care. Access is only granted to authorised healthcare professionals involved in a patient's care and is managed in accordance with NHS information governance, confidentiality, and data protection requirements.
