South Tees Hospitals Nhs Foundation Trust

The reason for this site accessing patient GP records is:

A shared care record provides health and care staff with a selected view of the patient information held within different local health and care settings.

Where a shared care record has used GP Connect an up-to-date view of the medical record held at the patients' registered GP has been provided. The shared care record user can access information such as the list of current and historical GP prescribed medications, recorded allergies and a record of recent GP encounters – alongside data from other sources such as local hospital trust and social care systems.

Additional Information:

We are commissioned by the NHS to provide acute and community services. We straddle two ICBs, Northeast and North Cumbria to the north and Humber and North Yorkshire to the south. Most services are commissioned by the Northeast and North Cumbria ICB and the Humber and North Yorkshire ICB. We provide all acute services across our two hospital site, We are an anchor tertiary provider delivering world-class cancer, cardiothoracic, spinal, cochlear implant, neurosciences, gynaecology and other highly specialist care for patients across the region – and one of only three hospital trusts in the UK operating three robotic surgical systems. Our major trauma centre sees half of all trauma cases in the North East and Cumbria. We also provide a number of community services. Our particular focus is for the UTC unit at the Friarage Hospital and the major trauma centre at James Cook, where access to the full patient record and history will be invaluable for treating patients safely. But we see access to the YHCR supporting patient care throughout the patient’s journey, including in the community.
