The reason for this site accessing patient GP records is:
A shared care record provides health and care staff with a selected view of the patient information held within different local health and care settings.
Where a shared care record has used GP Connect an up-to-date view of the medical record held at the patients' registered GP has been provided. The shared care record user can access information such as the list of current and historical GP prescribed medications, recorded allergies and a record of recent GP encounters – alongside data from other sources such as local hospital trust and social care systems.

HealthBus is a CQC-registered charity that brings specialist healthcare directly to people experiencing homelessness to reduce inequality across Bournemouth, Christchurch and Poole (BCP). We support adults who are rough sleeping, living in temporary or emergency accommodation, or facing severe exclusion from mainstream services. Many experience poor physical and mental health, trauma, stigma, digital exclusion, poverty, substance dependency and barriers to registering with or accessing healthcare. Our core purpose is to bring compassionate, trauma-informed support directly to people at the point of need, building trust where services have often failed. Through outreach, drop-in support, health engagement, social prescribing and strong local partnerships, we help people overcome barriers, reconnect with care, improve stability and move towards recovery. HealthBus is here to ensure that people experiencing homelessness are seen, valued and supported with dignity. HealthBus is committed to placing patient safety, dignity, and confidentiality at the centre of everything we do. Access to the Dorset Care Record (DCR) would help ensure that individuals experiencing homelessness and severe exclusion receive safer, more coordinated, and higher-quality care. This is particularly important for people who may not have consistent contact with healthcare services and whose medical information may otherwise be incomplete or unavailable at the point of care. The DCR enables authorised healthcare professionals involved in a patient’s care to securely view relevant health and care information, such as current medications, allergies, and recent treatments. For HealthBus patients, this can reduce the need to repeat their history multiple times, minimise delays in receiving care, and lower the risk of errors. HealthBus works closely within a multi-agency network, including primary care, hospitals, mental health services, housing providers, safeguarding teams, substance misuse services, and voluntary sector organisations. Access to DCR would strengthen this collaborative approach by allowing appropriate, real-time information sharing between services. This ensures that care is coordinated, consistent, and responsive to the full range of an individual’s needs, rather than fragmented across different providers. Patients will always be informed about how their information is used, and consent will be sought before accessing their record, except where there is a lawful basis such as significant risk or safeguarding concerns. Individuals can also choose to opt out. HealthBus maintains high standards of information governance, ensuring all data is handled securely, lawfully, and in line with national guidance, supporting better outcomes and safer care for vulnerable people.
