The reason for this site accessing patient GP records is:
Care Home/Hospice/Domiciliary Care/ Residential Care using GP Connect to securely view a resident's medical records which are stored at their GP practice.
Registered clinicians (for example, nurses) can access the full medical records of residents under their care. This helps them to deliver appropriate and timely care and treatment.
Non-clinical carers (for example, the care home manager) have limited access to residents' medical history to be able to safely provide continuous care. Some information they can view includes:
- Prescribed medications
- Allergies
- Recent interactions with GP
Further queries about the reason for a patient's record being accessed should be directed to the care home manager or equivalent.

Beaumond House Hospice Care is a registered charity committed to providing supportive palliative and end-of-life care along with emotional and spiritual support to patients, their families and their loved ones across Newark and Sherwood, Mansfield, Ashfield and Bassetlaw. Through our In-patient beds at the hospice, Hospice at Home services and the Integrated End of Life Care Hub, we help people receive personalised care and support in the place that matters most to them. We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources, services and self-help groups across Newark & Sherwood and the wider North Nottinghamshire area.
