The reason for this site accessing patient GP records is:

Care Home/Hospice/Domiciliary Care/ Residential Care using GP Connect to securely view a resident's medical records which are stored at their GP practice.

Registered clinicians (for example, nurses) can access the full medical records of residents under their care. This helps them to deliver appropriate and timely care and treatment.

Non-clinical carers (for example, the care home manager) have limited access to residents' medical history to be able to safely provide continuous care. Some information they can view includes:

- Prescribed medications

- Allergies

- Recent interactions with GP

Further queries about the reason for a patient's record being accessed should be directed to the care home manager or equivalent.

The shared care record user can access information such as the list current and historical GP prescribed medications, recorded allergies and a record of recent GP encounters – alongside data from other sources such as local hospital trust and social care systems
